The cable that broke in the cable car accident in Lisbon did not meet the requirements

Recently the investigation into the accident that happened on September 3rd, where 16 people died, released the first preliminary results. Although it appears that the maintenance was done correctly, the cable did not meet the requirements for passenger transportation.

According to this report, there have been mistakes in the acquisition of the materials and the internal control mechanism didn’t detect the problem. For me, this is unacceptable. How is it possible that the most critical part of a cable car was not checked and rechecked several times before installation? Who is responsible for this? No one noticed that during maintenance? These are questions that the investigation should address, and those responsible should go to jail. This shouldn’t have happened.
